I'm assuming you're using 4.x.
This can be done by adding the counters to the FileSystem monitoring template, after which, you can add thresholds and graph definitions.
Note that source name has to be exactly as you see it in Windows perfmon, and that the leading back slash is mandatory. See attached image.
